I'm a former competitive runner (15:30 5k/32:30 10k) trying to get back in shape after a 12 year
lay-off. I've been using a combination of biking and running, with a speed work out each week (most
recently a 400 ... 1km ladder). All was going well until I got a tender plantar's wart *and* plantar
fasciatis. I recovered from that, but a lot of my involuntary compensation (esp. in the am) involved
walking on my tip toes. I ended up with a sore achilles in the process. At a race this am, I was
running through the pain for the first 1.5 miles when suddenly I had a huge shooting pain in my
achilles. I stopped and trudged back to the start. There was no pop or sound when this happened, I
can stand on my toes pretty easily (small pain), I can point my toes and bend my feet. There's
noticable swelling, loads of dull pain, and stiffness (though no sign of separation). This was the
end of my season, so I have no trouble stopping running. I'd like to continue riding. Did I rupture
my achilles? Aside from RICE and stretching and working back into this easily, is there anything
else I should do?

A *sudden* huge pain in the achilles is often a sign of a rupture, but since you can still stand on
your toes and do other movements, it most likely isn't a total rupture of the tendon. I would say
you probably have a partial tendon rupture. Different people respond differently to this kind of
injury but in general, I'd ask you to consider the following:
1) RICE as you say, especially: REST - the key, I'd rest for at least a week from all activity, and
do little or no stretching at first, be *very* careful
2) Strongly consider making an appointment with an doctor or physiotherapist experienced and
interested in running injuries. You might have a serious achilles injury, and professional advice
will be worth the time and money.

I feel like such a dope. I was wrapping an ice pack on the painful area this morning and realized
that I'm not having pain or swelling in my achilles at all. In fact, the hard knot with pain is
about four inches above the achilles, squarely in my calf muscle. Now, in part one could reasonably
conclude that the most important thing here is for me to see a doctor, as I'm doing such a miserable
job in identifying the problem area. That conclusion aside, is this a calf muscle pull? More
optimistic prognosis?

It sounds very similar to an injury I had a year ago when I took up running.....like you I thought
it was Achilles when it happened.

However it was a pull to the soleus muscle (deep in the calf). 10 days rest and 2 sports physio
sessions got me back to gentle jogging, then running another 10 days later.

I'd suggest going to a sports physio and getting a proper diagnosis, then you can decide a plan!
