Exercise and Health Survey for California Residents:

Researchers at the University of Colorado want to learn about your preferred
type and frequency of exercise and how restrictions on your exercise would
affect your daily life. Your responses will be used to better understand the
importance that exercise plays in your life, whether you exercise a lot or a
little. The survey will take about 20 minutes to complete. In return for
completing the survey, you will be entered in a drawing to win a $100 American Express gift card.

Participation Agreement:
Your participation is voluntary and very much appreciated. You have the right to withdraw your consent or discontinue participation at any time. You also have the right to refuse to answer any questions for any reason. You should know that there are no foreseeable risks associated with your participation. If you choose to participate, we will not reveal any of your personal information, nor will we reveal any of your responses. All completed surveys with an e-mail address will be entered in the drawing to win the $100 gift card. Your e-mail address will not be seen by anyone except the primary
researcher. Furthermore, the list of e-mail addresses will be destroyed
following the selection of the winner.
