aftermarket braze-ons?

Try asking over at Quad Cycles in Arlington Heights. Rustem, the owner,
was chief mechanic of the Turkish International Racing Team and really
knows his stuff. He should have a pretty good idea of where you could
get the work done.

Call a few local bike shops and ask them for recommendations.

When I had this done recently, I paid maybe $15-20 per pair? I don't
have a hard number because I had the frame aligned at the same time,
including one major twist, and was given a lump sum total, but I think
that was the original estimate over the phone. This is in the San
Francisco area.

You are planning to repaint, yes?

Indeed. Got the frame largely stripped (perhaps one more session with the remover), and
figured now's the time to do it.

Peter Mooney at Belmont Wheelworks does this type of work. Also, Hot Tubes
in Worcester can do it, and repaint at the same time.
