The generic explanation to your question is: "tire sealant action is not a precise science".
There are so many factors in play when a puncture occurs (quantity of sealant present in the tire, size-geometry-position of the puncture, tire type/thickness, humidity, temperature, inflating pressure... and I could continue!) that having a sealant working 99% of the times is really an impressive result.
