Chain jumping off the large chainring

Anyone know what the likely cause of a chain jumping off the large
chainring and ending up on the pedal side can be caused by? This has
happened twice over the last week during up-shifting from the middle to
the large chainring (Veloce FD). I have not touched the various setting
except for adjusting the cable tension much before this.

Could this be caused by excessive cable tension? Or is a slight turn of
the outer position limit screw called for?

I think just adjust the limit screw. If the cable was too loose before,
maybe the lever took up just enough to get you onto the big ring but no
further without the limit screw coming into play at all, so it could be
some way out of adjustment.

The most obvious symptom of excessive cable tension would be if you
can't get into your smallest ring even with the lower limit screw backed
right off.
 
Common maintenance issue - just tighten the outer limit screw on your
front derailleur 1/4 turn and retry; sometimes takes a 1/2 turn. Zinns
or Bicycling Magazine's maintenance books or parktool.com are good
resources for stuff like this.
