When I got the frame & forks (1" steerer) it had the Chorus headset already fitted. However the forks would rattle in the frame when assembled. I took it to my LBS (normally very good) and he said that all bits were present and once I built the bike up (i.e. put the stem on) the play would disappear.
Not so.
So I looked on the Campag website (tech specs - spare parts (PDF) and to my amusement I noticed that different production years for the Chorus headset had different internal components! So I opted for the oldest year available (as the frame is old too!) on there to order what I think was missing (http://www.campagnolo.com/pdf/spares98-A.pdf page 8, I ordered the plastic centre HS-HD002 as seen in the first pic above with the rest on the headset).
This has stopped the play, but has made the steering very heavy. Also the sections of the headset do not close up snug anymore (pic 3). - - - - - - - - - - - - - -

Are you sure the bottom and top bearings aren't switched? The bottom is the larger set, top the smaller (diameter balls).

Update: It works! The frame (with headset) was indeed supplied with the lower and upper bearings switched. My friend is very grateful for the info.
