coaster brake not engaging

Does this happen in all three gears or just one of the three
gears? ISTR my 3-speed coaster brake hub behaving the same
way, I had to backpedal a fair bit when braking in first
gear. Unless this is accompanied by strange noises or
reduced braking performance, I wouldn't worry too much.

Do you happen to know who made your friend's 3-speed coaster
brake hub?

A loose bearing adjustment can diminish brake response. Some
three speeds lose braking in neutral (TCW series)

Oh, you mean "doesn't always engage brake"? Look at the
retarder assembly. Or a loose bearing adjustment.
