I just recently did this. Had been riding clipless for a month or so, never had problems and I live in San Francisco (frequent stops).
I was finishing up a quick 10mile ride around the park and decided to full charge up the hill to my gf's house. At the top of the hill where her house is I pull onto the sidewalk and unclip my right foot in preparation to lean right and catch myself... Only the sidewalk in front of her house is slanted down to the left. I was pretty winded from the hill and didn't notice until I started to lean left Needless to say I couldn't get my left foot unclipped fast enough and dooooown I went.
Thankfully I cushioned my bike's fall with my body so there was no damage except a scuff on my left pedal. The back of my lower right calf (right above the Achilles' tendon area) was not so lucky and had a run in my sprocket. Also thankfully, no one was around to whiteness me toppling over.
The worst part was having to clean out the chain grease from the cuts. Ouch. Oh, and explaining to my girlfriend why I was bleeding haha.
Lesson learned... unclip BOTH feet before coming to a stop, not just one. Just in case.
