Has anyone ever stomped off a pedal?

Yesterday, I was working on a friend's bike and was starting a test ride. A pedal come off the crank when I stood up and stomped down. The way the threads are boogered up makes it look like it wasn't screwed in all the way, though I did not notice this before the ride, though I called myself checking it. I had been working on a sticky shifter cable. Has anybody else performed an unplanned dissassembly of their bike using riding techniques? (Wrecking the bike DOES NOT count!)
 
I have cracked several plastic pedal bodies off of the spindle and once I had the spindle pull out of the threads on an aluminum crankarm - probably an assembly issue.
 
I snapped an aluminum crank arm in two pieces. The result was an immediate loss of control There was no further damage or injury luckily. I never had a pedal come unscrewed from the crank arm or had a pedal self-disassemble itself (although I have witnessed both forms of failure on several occasions).
