How do I change a gear cable on a Shimano (Ultegra) STI shifter?

Simple question (I hope) - my gear cable snapped right where it inserts into the shift lever.

How do I remove the cable head?

What's the procedure for threading a new cable into an Ultegra shifter?

I'm all up to date on derailleur adjusting, it's just the shifter-side that I'm having trouble deciphering. Thanks!

Shifter in the small chainring or small cog position,then punch it out from the side of the shifter the casing inserts into. How you thread in a new cable ought to be obvious from that position once you get the head out. Ya gotta depress the brake lever to expose the cable retainer.
