a friend's Bianchi Eros Donna comes with the following:

52/42/30 Sugino 5000 triple crank
13-26 Campy 9-speed cassette
Campy Mirage rear derailleur (medium cage, unfortunately):

The Mirage long is rated at 37 tooth capacity, with a 28 tooth max.
I BELIEVE the Sugino triple is 74mm-105mm-105mm bolt circle, which
should allow a conversion to a 24-38-48 in the front.

This is a derailleur load of 48-24+26-13 = 37 teeth, matching the
capacity.

Anyone see any issues with this? Can I convert the medium cage to long?

No need.

40-teeth total-difference is the point where it's worth getting a Campag
Long cage, IME. Below that, a Medium will cope, as long as chain length is
not excessive and the granny ring + smaller rear cog combinations are
avoided.

No need to get the long cage, the setup you suggest will work fine.
Even a 52/42/30 with 'medium' cage and 13-28 is common and no big deal.
The 'capacity' numbers are a conservative and rough guide.

Campagnolo Veloce 9 speed cassettes come in 13-28 and 14-28. Might be
worthwhile to get a bit lower gear. $40-50 for new cassette. Or you
can buy the QBP 28 tooth single cog for about $28 and replace the 26 on
your Mirage cassette. Make it a 13-14-15-16-17-19-21-23-28. I did
this back in 1999 when the QBP 28 cog only cost $20.

Your triple crank most likely has 130mm outer and middle rings and 74
inner ring. Replacing outer and middle rings gets costly. Inner rings
are about $10. A 26 inner ring will work satisfactorily with a 52
outer ring and a 13-28 cassette on a medium Campagnolo rear derailleur.
A 24 inner ring is too small and the medium cage will not wrap up
enough chain to use any but the biggest 3-4 cogs.

A 26x28 is 25 gear inches. A 30x26 is 31 gear inches. Enough lower to
notice.

The 'problem solver' 28t 9s cog is no more from QBP-
