Pannier Racks on a bike with disc brakes

I've just upgraded the brakes my On-One Inbred, from V's to Hope mini's, only to now realise that my standard pannier rack doesn't fit!

I’m about to go touring for a couple week, so any suggestions on how to get around this would be appriceated. I'm aware that I can use a seat post mounted system, but I'm a little concerned about how little weight they can carry.
