Yep. Both.

If it is a Shimano shifter, flushing the overly complex way too small
internal bits actually seems to help. Except for Bill Cotton, almost no
one opens the watchlike things.

Changing the wire position from inside to outside is done in certain
applications for exactly that reason. Using a Shimano Road Left shifter
with reverse anchor position on a Shimano MTB front changer can give a
faster shift at the expense of more lever pressure. Santana did that.

With Ergo of course any front shifts well.
