Sports Optical is the mac daddy of prescription cycling eyewear. I have Rudy Kerosenes from there, with three different sets of prescription lenses. Love 'em: no freakin' inserts.

A Sight for Sport Eyes has lots of different styles in prescription glasses, especially Rudy Project. I'm using their Horus model with the direct prescription lens in it....quality is excellent, as is the price.

IF you don't mind incerts then there is now better for protection than the ones used for the military now. They will stop a 12 guage shotgun pellet from 25 feet.
Revision Eyewear, ESS ICE, Wiley-X, Body and other as longs as they are Z87+ rated. Look at sites like Ranger Joes, US Cavery, Blackhawk Tactical
