You should possibly try a different brand. I find Shimano far too
narrow so to get an appropriate width I have clown-type extensions at
the front. Or rather I would, if I didn't use Diadora shoes that fit me
much better.

My Shimano sandals are a different matter, as there's no upper to
constrict my feet and I take my usual(ish) size 43 in those.

I have some Akito motorcycle boots that are size 47 - allegedly one size
bigger than I need and I have a monumental battle to get them on yesterday -
the first time that I'd ridden my motorcycle for an appreciable distance. I
don't have a problem with my Vans SPD trainers and Northwave MTB SPD size 45
cycling shoes.

I just wish that footwear manufacturers would standardise their sizes

I'm an 11, but wearing size 12 Specializedz, and they're quite snug. I'd
recommend visiting a LBS and trying a pair on. They might even price match
wiggle. Maybe.

Shoes is one of those things you have GOT to try on. Even then you can
have problems. Go in the afternoon and wear the thickest socks you
want to wear with your new shoes. I've got a pair of Nike casual SPDs
that I can only wear with thin socks despite trying them on in the
shop and buying a couple of euro sizes bigger than I thought I was.
