That's a pretty small keg. I'd hate to have to wait for beer to
come out of that.
I think it's for corked vinegar (probably sherry)or medicine.
You drill through the cork and leave it installed in the bottle.
The valve/top is the dispenser.

It's a wine tap. It's intended mainly for serving very old bottles of wine
where a regular corkscrew might tear apart the cork. It gets inserted right
down through the cork.
Not all that useful, but at that price I'd go for it. Especially with that
cool carrying case. I'll trade you my cheese straightener for it. Or maybe
even my antique basil flayer.

That's the most convincing explanation yet.
It explains the fancy box. I don't buy wines
that require that kind of treatment, though.

That box is very typical of precision machine tool cases from the
early 1900s ie. Browne & Sharpe, L. S. Starrett, etc.
I don't think that thingie has anything to do with wine or any other
beverage... to me it appears to be some kind old medical instrument,
some sort of gas valve, something an MD or DDS would use in
administering an anesthetic

It is a surgical tool. It is used to measure the thickness of the skulls of
people who spend all day cruising through Ebay to find things they can't
identify.

Too bad the box it's in has nothing to do with the instrument
itself. The indentations in the box are clearly not for that
device, which has been put in randomly (facing left or right)
judging from the wear marks on the inside of the lid.
Clearly, that box was not designed for that piece.
The only thing I wonder about the wine/cork explanation, is that
this device seems to blunt on the end to drill through fragile
cork. It seems it would tear up the cork more than the
traditional corkscrew. And where does the excess cork go? Into
the wine, possibly clogging the dispenser.
It seems it would have to be pre-drilled for that device to be
useful as a wine dispensing tool.
