This is a follow up survey for doctors/dentist/therapists, and caregivers only.
As a student at Champlain College, in Burlington, VT. I am conducting a survey to identify gaps in
patient/doctor relations.
The survey consists of 35 statements which best need answering. Do not take this survey if you do
not provide a medical service, such as a doctor/dentist/surgeon, or patient care provider such as a
Physical Therapist, RN, and Chiropractor.
This is a confidential survey- No information could be used to identify you. The survey is designed
to collect demographical data on the makeup of medical service providers within a region, to
understand existing, and to develop ways to address those needs.
