I'm putting the finishing touches on my new fixie and having a small problem with the brake lever/cable. I'm running a Tektro aero bar lever that plugs into the end of the cow horn bar. The lever takes a brake cable with the barrel end rather than the mushroom end on a regular road lever. The cable end I got at the LBS seems a little too small for the lever and twists in the stop when the lever is pulled.
Are there different sized barrel shaped ends; like maybe BMX vs. MTB? Any other ideas what's up with the twisting cable end?

Just a standard MTB type brake cable end. Connect the brake and then try to rotate the end so it sits where it should. The tension of the brake should keep it there.
