Longer road hub rear axle?

Yes, you can substitute an MTB axle. I've done that on a couple of Shimano
road hubs to change them to 135mm spacing.

American Classic also make good quality CrMo replacement axles.

Do you have to use the MTB cones and bearings, or can you just swap out
axles and use all the road "stuff"?

You should use the original cones that came with the hub. I've done
this on Shimano 105 and Ultegra hubs in order to widen them to 135mm
width. The new axle and a 5mm spacer are all you need.

try harris cyclery's site-search into parts catalog then see Wheels
Manufacturing axles. shim's are 10mm x1. if using a rack and touring
consider the extra long axle. one belts the assembley back together,
on the road, without regard to micro adjustments. the metal is
excellent and with less than normal intelligence can be starightened
with a vise and tool, hammer or pipe fit over. and felt pen and level
for straightness checks off course
