I have been having a specific shifting problem with my XT derailleur on my Trek Fuel 98. Despite all the cable tension adjustments and H and L limit screw adjustments, the chain hesitates when going from the largest rear cog to the second largest rear cog. It finally goes on when I ever slightly hold the shift lever down. In other words, I'll click and then hold down slightly and the shift is made. All other cogs shift with crisp precision. Anyone with any advice along these lines would be deeply appreciated.

Try replacing the shift cables and the housings. That always seems to resolve the "mystery hesitation" shifting problems I sometimes get.
Check your derailleur hanger to see if it is bent. The chain is "steered" among the cogs by the upper guide wheel of the derailleur. If the hanger is bent, the guide wheel, which is riding very close to the largest cog when the chain is on that cog, may be guiding the chain toward the largest cog when you attempt to shift to the next cog.
Also check the B screw adjustment which controls how close the upper guide wheel gets to the largest cog.

I recently put a brand new hanger on and have adjusted B screw as well. But not with chain on largest and second largest cogs. I'll give this a try. Thanks!
