It's a screening test, for exposure, not use. It depends on
the drug residue discovered. Marijuana exposure could be due
to inhalation, or second-hand smoke (which might also be
problematic). Evidence for opiates would more likely
indicate use. You can be sure it's not the only test used,
it's just the least invasive place to start looking.

It might, but no one has proven that it does. Even hair that
is only half an inch from the head has been exposed to the
external environment for a whole month, so not only does it
not reflect anything about the current state of the body, it
has confounding effects from where the body has been.
