That's one answer, there are a couple of others:

In road-racing contexts, it means "Off The Back" i.e. a rider who has lost contact with the main
pack, and who is basically out of the race as a result of losing the pack's slipstream.

It's also a bicycle brand, trademark of Boston's Charlie Hamburger, who was an early (1940s and 50s)
U.S. importer of high end bicycles and parts. His brand was "Only The Best." I own the last OTB,
from 1957.

There's also "On The Bubble", as in "Mario definitely will ride the Giro, Fabrizio's recent injury

That's where I saw the referance to OTB - I didn't think it was a brand name - I thought it was a
type of bike.
