Replacing derailleur hanger

If stuck, copy the old one.
Most hangers are a plate of metal which hooks around the axle. It has three
holes in it. Two are simple screw holes with countersinks. One is difficult
as it has a tapped thread to take the derailleur.

Find someone with a metal working hobby.
Its a bit of drilling, filing and one tapped hole in some plate, so one
needs hacksaw, files, drill bits, countersink, and tap handle. A drill press
is useful. Nothing else.
The standard derailleur hanger thread is M10x1mm. A carbon steel tap in
this size costs about £4 - if ordering one, get a taper tap for this job.
The drill size for this tap is 9mm.

somebody with purchasing as a hobby will probably get a better result

But is the step actually necessary ? I'd have to look carefully to decide.
If it is, then yes, milling comes into it as well.

I guess you could make the hanger out of steel and put a nut on the
back, but generally the step allows the unsupported part of the hanger
to be thicker so it's stiffer and you have a decent length of thread.

you could try walking into an Italian LBS and have the owner take one
off a brand new frame and fit it for (almost) nothing, worked for me
;-)
Since bending the last one (note to self, fords may still be wet in icy
conditions but the road either side isn't) and replacing it I carry the
old one everywhere.
