A subject dear to my heart!
I own a Canon 400D with a 18-200 lens, if the camera bag is too big I tend
to get into the habit of not taking the camera. So I wanted a bag that could
either be carried via a shoulder strap or worn round the waist.
When I first started looking for a suitable bag I only had the camera but to
make matters worse I recently bought the Canon battery grip which makes the
camera a fair bit bigger. Back to square one with what seemed like even less
choice.
After much searching I found a Tamrac that fits. Their Pro or Zoom 7 is
designed to take a camera with zoom lens and a battery grip! I ordered one
and it arrived last week.
It's exactly what I need, it has belt loops, the loop has press studs and
Velcro so you don't have to thread it onto the belt, and can also be hung
off a rucksack if required. It comes with a removable shoulder strap too.
The do a range of sizes including one that's small enough for your needs.

As someone else mentioned, they also do a 'utility' belt that you can hang
the bag on along with a variety of other bags for lenses, drink, etc.
