  1. What is the standard method of listing weight, sets, and reps? If I say
    125 x 20 x 15, what I mean is 125 pounds, 2 sets, 1 set at 20 reps, and
the second set at 15 reps. Also, I did call heal raises by the wrong
name. I posted this here, because my other posts in the already
    established topic did not appear.

    There are different methods.

You might want to record this lift as

    2 X 20, 15 @ 125

    or

    125/20, 125/15

    or

    125 X 1 X 20
    125 X 1 X 15

I use, sets, reps, weight in my log
3 sets of 8 reps using 315 would be: 3x8x315

I put the weight last because sometimes I don't use any weight,
like for situps or crunches; 1x25 or 2x20.

    I like others I have seen used: 3x8 - 315 or 315 - 3x8
    I see this quite a bit; 135/8 225/8 315/5x2 sets 405/5
    (weight/reps x sets)

I would write your example as: 1x20x125, 1x15x125

I write them out as I do them, e.g., if I do 300 lbs. for 5 reps, then I
    write 300 x 5, and if I do that for 2 sets, then I write 300 x 5 x 2. I
believe a lot of Americans doing it the opposite way, sets first and
reps second. As long as you know the convention you're using, it
    doesn't matter.

I'll add that some of us vary the number of weights in our sets so a
more likely entry for me would be something like 300 x 5, 3, 4 to
indicate the reps in each set. I have no problem writing out 300 x 5,
    5, 5 if that's what I do.

I always write the weight first followed by the number of reps and
start a new line for every set. The only exception to this is when
I'm doing speed bench on the weekend. Then I do 9 sets of 3 reps and
vary my grip every 3 sets. Sunday's workout was like this:

    95X15
    135X10
    185X5
    200X3X3 (competition grip)
    200X3X3 (medium grip)
200X3X3 (index fingers inside the knurling)
    30 seconds rest between sets

    Same here,

    ex. Bench 405 x 6 x 6 415 x 6 425 x 5 x 5F (i wish!)
    Incline 315 x 6 x 6 x 6 x 5B
    Side-lats 50 x 6 x 6 x 6+

I also use codes after the reps sometimes,

F = failed on next rep attempt
+ = cheated out a few more reps/help from spotter
B = barely got this rep - probably cheated a bit

If I don't get a rep legally (but without a pause, normally in the
case of the bench), I write the last rep down as 7/8 or 3/4.
